Jack R. Black passed away on June 16, 2022 after years of various illnesses. Jack was born on June 6, 1949 to Jack and Dorthy Black in Nephi, Utah.
Jack proudly served his Country in the Vietnam War from 1967-1968.
Surviving Jack is his wife, Karen. They were married in December of 1980 and together for 41 years. He was preceded in death by his parents.
The family would like to thank Elevation Hospice Care for their services.
